Mandatory questions required by DSS on enrolment form

The Enrolment Form Guide is available on the ETO Forms and Information section of the HIPPY Online Library and explains why each of these questions are asked. 


The following enrolment form questions are required by DSS.


  • Date of birth (DOB)
  • Home address
  • Gender
  • Is the child Aboriginal and/or Torres Strait Islander?
  • Country of birth
  • Cultural background
  • Main or other languages spoken
  • Does your child experience any challenges?
  • Has your child been diagnosed with Autism Spectrum Disorder (ASD)? Or, do you think your child may have ASD?
  • Are you in paid employment at the moment?


Questions relating to the Children, Families and Communities Outcome Areas on the enrolment journey forms are also required by DSS. However these questions are program-specific.
